The Program Director (Memory Care Director) is responsible for on-site coordination of all clinical, programming, and staffing aspects of the Program at the Community. This includes pre-admission assessments, programming, staffing, staff training, and supervision. The Director assists residents and their families in maintaining physical and emotional health, liaising with hospital personnel, physicians, community organizations (including the local Alzheimer's Association Chapter), and other health service agencies to provide comprehensive care. Responsibilities also include hiring, evaluating, coordinating, motivating, monitoring performance, scheduling, and supervising department staff according to company policy. Recommendations regarding staff evaluations, merit increases, reprimands, disciplinary actions, and terminations are made to the Executive Director. This position is for units with thirty (30) or more residents.

Responsibilities:

  • Plan, develop, organize, implement, evaluate, and direct the unit, programs, and activities in accordance with federal and state policies governing dementia care, including personal on-site oversight.
  • Interview, hire, evaluate, coordinate, motivate, monitor performance, schedule, and supervise department staff, focusing on qualified candidates interested in dementia care and management. Make recommendations to the Executive Director regarding staff evaluations, merit pay increases, reprimands, disciplinary actions, and terminations.
  • Coordinate departmental schedules to ensure adequate staffing aligned with needs, company standards, policies, procedures, budgets, and state law.
  • Oversee continuing education programs for caregivers serving dementia residents, in compliance with regulatory requirements.
  • Conduct onboarding for new department members to ensure familiarity with goals, policies, and procedures.
  • Arrange for coverage during staff absences through delegation or personal duty completion.
  • Act as Vibrant Life Coordinator until a dedicated Vibrant Life Assistant is hired, once the unit/community has sufficient residents.
  • Ability and willingness to operate community vehicles and obtain a commercial driver's license if required.
  • Conduct and coordinate assessments for residents, including pre-admission evaluations, care plans, and regular re-assessments, in accordance with current regulations and policies.
  • Interview potential residents and families, complete assessments, obtain medical records, and fulfill other requirements for admission as per Title 22 regulations.
  • Monitor residents' physical and emotional health through ongoing observations, alerting the Executive Director and families to any changes. Conduct monthly service plan meetings.
  • Ensure timely and accurate data entry into the care system, maintaining all documentation and records.
  • Coordinate nursing and other services to ensure seamless resident care.
  • Serve as a resource for residents and families, contacting physicians and family members for specific orders and representing the property professionally.
  • Conduct community tours and promote the community to prospective residents and families.
  • Support referral development and maintain high occupancy rates.
  • As a mandated reporter under California law, report any suspected abuse of dependent adults or elders.
  • Participate in property inspections and surveys by governmental agencies.
  • Maintain a safe environment, adhering to safety standards and coordinating special safety provisions for residents with specific needs.
  • Perform fire and safety procedures as needed.
  • Maintain inventory of supplies.
  • Participate in daily team meetings to communicate key issues.
  • Assist in departmental budget preparation and monitor costs daily.
  • Maintain current knowledge of dementia-specific care practices and industry developments.
  • Perform clerical duties related to the position.
  • Foster teamwork and professionalism in daily activities and interactions.
  • Support a positive public image through actions and attire.
  • Perform other duties as assigned by the Executive Director.

Qualifications:

  • Must be at least 18 years old.
  • Preferably, have two years of college education in gerontology, nursing, social work, or a related health or human services field, or hold a professional license.
  • Prefer two (2) years of experience providing residential care to the elderly or similar experience with dementia care programs.
